Your source for FREE developmental questionnaires to celebrate your child’s developmental milestones, and activities to support healthy growth for children ages birth – 8 years old.

Benefits of Child Development Questionnaires

As your child grows, they develop different skills and, in the process, reach what is known as developmental milestones. Developmental milestones vary for each age range and cover four categories of a child’s development: cognitive, communication/language, social and emotional, and fine/gross motor.

Reaching developmental milestones is a part of growing up and something to be celebrated!

By completing regular developmental questionnaires, you can celebrate milestones as your child reaches them or receive support if your child is not yet meeting specific milestones. The earlier a potential delay is detected the sooner interventions can be put in place to support your child.

Types of Questionnaires

The Ages and Stages (ASQ-3) Questionnaire assesses the developmental performance of a child in the areas of communication, gross and fine motor skills, problem solving, and personal-social skills.

The Ages and Stages Social Emotional (ASQ-SE) Questionnaire measures self-regulation, compliance, social-communication, adaptive functioning, autonomy, affect, and interaction with people.

The Pediatric Symptom Checklist (PSC) is a brief questionnaire that helps identify and assess changes in emotional and behavioral problems in children.
